| 2008 will be the celebration of the
8th annual Tournament. This year the Tee Up for Life Golf
Tournament will be held on Monday October 27th, the pairings
party is held Sunday October 26th.. The tournament is held at
the Country Club at Heathrow, in Heathrow Florida.

| The night before the Tournament there
is a Celebrity Pairings party hosted by Chris and Amy DiMarco.
The pairings party also has a silent auction, entertainment,
cocktails and buffet for the sponsors, players and guests. |
| The tournament on Monday starts with a
golf clinic that is hosted by Chris DiMarco, and stars Nancy
Lopez and Jay Golden, after which there is lunch and then off to
the course. Following the tournament there is a silent auction
and cocktails, and then dinner, awards, prizes, and a live
auction! |

for the start of the event in an effort to try to allow time for
those that may want to have time to pick up spouses or guests,
clean up, etc. before the evening festivities. |

the address provided along with your payment. Online
registration is not possible to ensure equitable reservation of

| Shamble - each player on
the team will hit a tee shot, the team will select the best
location, and each player will play their own ball according to
stroke play rules from that
location to determine scores |
| 2 Best Balls of Five -
Combined Gross, and Net (percentage of handicap TBD) |
| Fivesomes - 4 golfers plus
celebrity |

Players with
handicaps of 5 and under will play from the GOLD tees, those
with handicaps 6 and higher will play from the BLUE tees. |

In the event
of ties - score cards will be matched starting with #18 and
working back |

All players
GROSS scores are to be recorded on the score card - the NET will
be determined by tournament officials |

par 3's, - par 3 #12 can use Chris DiMarco's Tee shot for low

| The event will be held
rain or shine. Please register and plan to play on time
even in the
event of rain. |

In the event of extreme
weather at the time of the tournament, the tournament officials
will determine if play is to commence. Every effort will be
made to play as we are unable to reschedule the event due to the
scheduling of Chris DiMarco and our Celebrities, as well as
logistics with the golf course and sponsors. We appreciate your
